Improving Care for Patients with HIV in Uganda and Leveraging These Lessons to Strengthen Systems to Manage Chronic Conditions

The USAID Health Care Improvement Project (HCI) is working with health providers in Uganda to help health systems change from the current model designed to deliver short-term care to one which can help people living with HIV manage their health at home. HCI is promoting the use of the Chronic Care Model, which is an evidence-based set of principles for improving chronic condition care. HCI is also encouraging providers to take the lessons learned from improving HIV care and apply them to care for patients with hypertension (HTN), diabetes mellitus (DM), and other chronic conditions. This short report describes the burden of HIV, HTN, and DM on outpatient clinics in a single district in Uganda, describes how facility staff improve HIV services, and how providers have taken these lessons from HIV care and applied them to improving care for patients with HTN and DM.
